The King's Daughters' & Health Services

All Hanover College student-athletes must complete a three-step process to provide information to be eligible to compete for the Panthers. The process covers both first-year and returning athletes.

First-year students must have attended a LEAP session prior to completing the forms. The LEAP session provides a student identification number and password for MyHanover. Each student's ID number also serves as the athlete ID number required in Step Two.

All forms must be completed between July 1- August 6.

Step One

After attending LEAP, first-year athletes login to MyHanover, click the Student tab, then click Student Health, then click on Sports Medicine Form.

Returning student athletes login to MyHanover, click on the Student tab, then click Student Health, then click on Sports Medicine Form.

Step Two

  1. Visit athleteconnection.net
  2. Click Register to create a new account.
  3. Enter information requested:
    1. Account group: 10471
    2. Account code: 3778387
    3. First name: enter your first name.
    4. Last name: enter your last name.
    5. Email: Use your Hanover account.
    6. User name: Hanover ID #
    7. Password: Something you will remember (we recommend your sport i.e. football)
    8. Click Register   .
    9. Write down your register information as you will be asked to complete this process each year you are Hanover.
  4. Once your account has been created, you will be taken to your profile page. Please complete the information on this page.
  5. Click Emergency Contact Tab and update information.
  6. Click Insurance Tab, update information.
  7. Click Notes Tab, update information.
  8. Verify that all information is correct.
  9. Click Save Changes.
    1. You will be asked to enter a 6-digit PIN. Note: the number does not matter
    2. You will be asked to enter your date of birth
    3. Verify that the bottom date is correct
    4. Click Apply Signature and Save
  10. Check that it says “Your changes were saved successfully” at the top of the page
    1. If it does not say this, click the Save Changes and repeat step 9.
  11. Log off (top right) if you are on a public computer.
  12. This website will not be available until July 1, 2011.

Step Three

Print the following forms, complete and mail to:

Also, bring your medical insurance cards or a photocopy to campus at the time of your physical.

Athletic Physical Schedule

First-year student athletes who are not playing a fall sport need to schedule a physical with Health Services between August 29-Sept. 9, 2011. Do not schedule appointments until you get on campus. You will need to consider your August Experience and class schedule before making this appointment. The Health Services phone number is (812) 866-7082.

The King's Daughters' Hospital & Health Services
